Privacy Statement & Educational Only Disclaimer
Privacy Statement
The Vanderbilt University School of Medicine respects your privacy. Any personal information collected from participants in online continuing education activities we sponsor will not be shared with any third party.
There are a few ways in which we collect data concerning your participation in online learning activities at this site. If a User Session has not started, when you (the user) click on the "Modules" button, you will be prompted to either sign in or complete a simple registration (first name, last name, username, password, e-mail address, occupation), if you have not registered previously. If you have already registered, we ask you to "log in" with your username and password. This is used to identify you so that you can pause and later resume your progress through the modules. This also allows us to create a unique record of your participation without knowing anything more about you. When you complete an activity, we will ask you to provide additional information if you want continuing education documentation of your participation in this activity: full name, degree, specialty, postal address and telephone number. This makes it possible for us to store a record of your participation in an electronic database, and it allows you to print a certificate. It also makes it possible for us retrieve your account information should you need to access your certificate. We also collect aggregate data related to participant responses to module questions, as well as responses to user surveys. (Completion of a post activity survey is required of those seeking CME credit documentation; completion of a follow-up survey is optional for those in this group.) Completion of both a post activity survey (activity evaluation) and of a 3-month follow-up survey are optional for those not seeking CME documentation. This information is only used to assess the effectiveness of the program and to identify and guide potential enhancements to the program and does not contain any identifiable personal data. We also count "visits" to pages (different parts of this website) in order to learn which pages are visited most often. As with any website, we know your originating service provider, such as America Online, or referring Web link. We may utilize your "point of entry" to determine the extent to which particular external web sites or other sources refer visitors to this site. At the end of the evaluation you will be asked if you would like to receive a limited number of CME e-mail notices, annually, concerning activities you might find of interest. From time to time you may encounter optional user surveys, which seek to help us better tailor CME resources and activities to meet the needs and interests of the professionals we serve. Please let us know if you have any concerns or suggestions regarding this policy or any of our CME programs or other services. We can be reached at info@improvementskills.org. Thank you.
Educational Disclaimer

Acknowledgments of Financial & In-Kind Support
We wish to acknowledge and express our sincere appreciation for support for original work contributing to the development of this project from a grant awarded by the Contemporary Education Initiative of "The Network" at the Center for the Health Professions, University of California, San Francisco. We would also like to acknowledge and express our gratitude for the unusual encouragement and support provided to this project by the Case Western Reserve University Division of Information Technology Services in the form of technical assistance and an unsecured loan, making it possible to initiate a major redesign and upgrade of the HISC Website and accompanying healthcare improvement skills web modules, in 2004-5. The Case Western Reserve University School of Medicine Program in Continuing Medical Education, the University of Missouri-Columbia School of Medicine, the Academy for Post Graduate Health Care Education, and the Institute for Healthcare Improvement all contributed a great deal of encouragement and in-kind staff time to the initial stages of this undertaking. In addition, IHI's New Health Partnerships project awarded a small grant making it possible to incorporate an example of patient participation in an improvement project in Module 2, Building a Team. Finally, we would like to acknowledge, here, the participation and significant contributions of two commercial vendors to this initial work, Medical Directions Inc., Tucson, Arizona, and Improvement Learning, Shaker Heights, Ohio.(Top)
